Housing Case Manager
Provide housing-focused case management services to homeless families in a Tier II emergency family shelter to rapidly secure permanent housing.
Works collaboratively with Westchester County Department of Social Services (WCDSS) and other service providers to help families achieve housing goals.
Provides comprehensive case management services to residents including bi-weekly ILPs.
Formulates a housing plan with residents to ensure a timely transition to permanent housing and appropriate usage of housing subsidies.
Refers residents to specific housing opportunities.
Conducts biweekly room inspections.
Provides crisis intervention as needed.
Participates in monthly Town Hall meetings.
Submits and tracks work order requests.
Assists residents with completion and filing of housing and rental subsidy applications.
Helps residents secure moving costs from DSS, including broker's fees, security deposits, first month's rent, and furniture allowance.
Links families with community-based providers that are able to meet their long-term social support and service needs.
Maintains accurate written record of services provided to families and all required documentation in accordance with HUD and WCDSS requirements.
Maintains demographic and other needed information regarding residents for use in statistical reporting through Client Track HMIS and other databases.
Submits accurate, timely reports as requested.
Other duties as assigned.
Bachelor's degree in social work or related field preferred. Strong computer, written and verbal communication skills required. Preferred experience in one of the following areas: case management, home visiting, homelessness, or child welfare. Understanding strength-based work and trauma informed care. Sensitivity to the needs of culturally diverse families. Fluency in Spanish is highly desirable.
