Position: Case Manager Supported Housing HV
Location: City of White Plains

Job Summary

Responsible for ensuring that all program participants receive services to promote personal and economic independence, develop skills for obtaining, maintaining, and retaining permanent housing, and live in well‑maintained units.

Location:

White Plains, NY (ZIP 10601)

Hourly pay: $26.45 – $55,000. Pay range based on experience and qualifications.

Schedule:

Monday–Friday, 10:00AM–6:00PM. Overtime eligible for FSLA non‑exempt employees.

Responsibilities
  • Adhere to and enforce program regulations, policies, and procedures, ensuring clients receive quality services in compliance with the program.
  • Screen individuals referred to the program and facilitate admissions, reviewing rules, rights, property, key distribution, and lease agreements.
  • Assist clients in developing Independent Housing Plans (IHP) by identifying housing needs and goals.
  • Provide information, referrals, counseling, crisis intervention, direct service, and advocacy services.
  • Develop and maintain a resource network for referrals related to client needs and collaborate with OMH, DCMH, DSS, Health Homes, property management, and other service providers to fulfill IHP and program requirements.
  • Assist clients with direct services such as budgeting, ADL skills, utility payments, and other appropriate support.
  • Conduct face‑to‑face meetings with clients no less than twice per month (or as program needs dictate).
  • Inspect living spaces regularly to assess ADL needs and program compliance, submit work orders to property management, and evaluate, document, and report physical plant needs.
  • Lead social/psycho educational and recreational groups directly or by engaging outside speakers.
  • Complete Foothold Awards and HMIS data and reporting within specified deadlines, ensuring resident files are current.
  • Mediates disputes between clients and property management regarding rental payments, ADLs, and neighbor relations.
  • Prevent loss of income from vacancy loss or unpaid rents, safeguards against DSS housing sanctions, and ensure proper use of company resources.
  • Prepare discharge plans, exit property reviews, and key collection.
  • Submit all internal and external reports, charts, and files in a timely and accurate manner to funding agencies and CHI.
  • Maintain clinical appropriateness and confidentiality at all times.
Qualifications
  • Education:

    Bachelor’s Degree;
    Master’s Degree in human services or related field preferred. Extensive experience may substitute for educational requirement.
  • Experience:

    Minimum two years in a related field such as mental health, homelessness, healthcare, or housing.
  • Communication:
    Excellent verbal and written communication skills; proficiency in English required. Bilingual Spanish is a plus.
  • Computer

    Skills:

    Basic competency in MS Word, Excel, Outlook, and internet use.
  • Math

    Skills:

    Ability to formulate simple financial budgets.
  • Physical Performance:
    Ability to make home visits, walk distances, climb stairs, and perform related physical tasks.
  • Reasoning:
    Ability to prioritize, make appropriate decisions, and exercise sound judgment.
  • Other

    Skills:

    Ability to relate to underserved populations, work independently or with a team, be highly organized, manage deadlines, multitask, adapt, and maintain high detail orientation.
