Role DescriptionWe are seeking a dedicated Primary Care Physician to join our community health team. The purpose of this position is Providing compassionate, high-quality medical care that includes diagnosing illnesses, developing treatment plans, prescribing medication, conducting exams, and counseling patients on preventative care, while working with medical, dental, and behavioral health teams to provide comprehensive care, maintain accurate medical records, and stay up to date on new medical and empirically based care practices.
Responsibilities- Conduct medical assessments, diagnose illnesses, and develop treatment plans.
- Educates patients on wellness, health maintenance, and disease prevention by providing materials and resources to the patients and families.
- Determines and works closely with care coordination team on which referrals are required based on examination and patient needs.
- Utilize electronic health record (EHR) systems efficiently for documentation and patient management.
- Collaborate with multidisciplinary teams including behavioral health specialists regarding patient care, assessment, and education needs.
- In rotation with fellow providers, participates in the after-hours call system to be alerted by triage nurse if a need presents.
- Works with the Medical Director and the Clinic Operations Manager in maximizing utilization of clinical staff in order to achieve a reasonable level of practice productivity.
- Participate in community outreach programs to promote health awareness.
- Stay current with medical research and maintain clinical competencies.
- Ensure compliance with healthcare regulations and standards
- Medical degree (MD or DO) from an accredited institution.
- Valid medical license to practice in Wisconsin and Board Certified in Family Practice, Internal Medicine, or Pediatrics.
- Current DEA registration number.
- Proven experience in primary care or family medicine.
- Proficiency with EHR systems.
- Strong communication and interpersonal skills.
- Ability to work collaboratively within a healthcare team.
- Commitment to providing patient-centered care.
- Knowledge of current healthcare regulations and best practices.
