BAYADA Home Health Care is seeking a compassionate and dedicated Occupational Therapist (OT) to join our Ocean County Visits team. As a Home Health Occupational Therapist, you'll play an integral role in helping clients maximize independence and improve their quality of life by:
- Providing skilled home visits within designated territories.
- Performing comprehensive evaluations of functional abilities, including ADLs, IADLs, mobility, cognition, communication, and home safety.
- Developing individualized treatment plans to improve functional outcomes and independence.
- Assessing home environments and recommending adaptive equipment and modifications.
- Collaborating with nurses, therapists, physicians, and other healthcare professionals to deliver patient-centered care.
- Educating patients, families, and caregivers on therapeutic interventions and safety strategies.
- Accurately documenting assessments, treatments, and patient progress using electronic medical records.
- Participating in interdisciplinary care planning to achieve the best possible outcomes for patients.
- Current New Jersey Occupational Therapist (OT) license.
- Graduate of an accredited Occupational Therapy program.
- Minimum of one year of recent OT experience.
- Prior home health experience preferred but not required.
- Strong clinical, organizational, communication, and problem‑solving skills.
- Ability to work independently and effectively manage time.
- Experience with EMR documentation preferred.
$60.00-$70.00 per point ($93,600-$109,200 annual earning potential)
