Pediatric Home Health Registered Nurse (RN) - PRN

Bright Star Care is seeking an experienced, compassionate, and highly dependable Pediatric Home Health Registered Nurse (RN) to provide one-on-one skilled nursing care to a medically complex pediatric patient in the comfort of their home.

This is a high-acuity pediatric case requiring 24/7 nursing coverage
, with a need for approximately 4-5 experienced PDNs to provide consistent care. The family has high expectations for clinical experience, professionalism, attentiveness, and the ability to work collaboratively within the home environment.

The ideal nurse will be physically mobile, alert and attentive throughout the shift, proactive, dependable, and comfortable taking initiative while providing exceptional care to a medically complex pediatric patient.

Key Responsibilities Direct Patient Care
  • Provide skilled one-on-one nursing care in the patient's home.
  • Perform comprehensive nursing assessments and ongoing monitoring.
  • Administer medications and treatments as prescribed.
  • Monitor and document the patient's response to care.
  • Recognize and promptly report changes in the patient's condition.
  • Remain alert and attentive throughout the entire shift.
  • Take initiative in addressing patient needs and maintaining a safe care environment.
Specialized Pediatric Care

Experience with medically complex pediatric patients and the following is highly valued:

  • Tracheostomies
  • Mechanical ventilation
  • Gastrostomy tubes (G-tubes)
  • Seizure disorders
  • Oxygen therapy
  • Other complex medical conditions and equipment
  • Ensure medical equipment is functioning properly and safely.
  • Follow emergency and individualized care protocols as directed.
Family & Care Team Collaboration
  • Work respectfully and professionally with the patient's family.
  • Communicate effectively with parents, caregivers, physicians, therapists, and other members of the care team.
  • Maintain a family-centered approach while respecting the family's expectations and routines.
  • Demonstrate professionalism, good judgment, and strong communication within the home.
Care Coordination
  • Follow and implement the patient's individualized Plan of Care.
  • Collaborate with the interdisciplinary care team as needed.
  • Communicate significant changes or concerns promptly.
Family Education & Support
  • Provide education regarding medications, treatments, equipment, and safety.
  • Support family members and caregivers in understanding and participating in the patient's care.
  • Demonstrate patience, compassion, and professionalism at all times.
Documentation & Compliance
  • Complete accurate and timely clinical documentation.
  • Maintain compliance with agency policies, state regulations, and nursing standards.
  • Follow HIPAA and patient confidentiality requirements.
Qualifications Required
  • Current, unrestricted Texas Registered Nurse (RN) license.
  • Current CPR/BLS certification.
  • Minimum 1 year of nursing experience.
  • Strong clinical assessment and critical-thinking skills.
  • Reliable transportation and valid driver's license.

    Ability to remain alert, attentive, and physically active throughout the shift.
  • Ability to work effectively and professionally with pediatric patients and their families.
Strongly Preferred
  • Significant pediatric nursing experience.
  • Experience with medically complex or high-acuity pediatric patients.
  • Private duty nursing (PDN) experience.
  • Home health experience.
  • Experience with tracheostomy and ventilator patients.
  • Experience with G-tubes, seizure management, and oxygen therapy.
  • Experience working collaboratively with families in the home setting.
