The Family Preservation Services Case Manager provides for the safety, stability and well-being of children and families by delivering and coordinating culturally competent case management services that are responsive to the unique needs of the family.

START (Sobriety Treatment and Recovery Teams) is a family-centered, collaborative intervention for families with CPS involvement and children 0-5 years of age, who are impacted by parental substance use. START services are provided to each individual and to the family as a whole, focused on strengthening the relationship between the parents and children.

Essential Functions
  • Provides for the safety, stability and well-being of families during the referral period.
  • Delivers culturally competent, strengths-based case management services to children and families consistent with evidence-based practice models.
  • Make decisions based on guidelines provided by the supervisor, family preservation standard procedures and policies, evidence-based practice model, and DCCCA organizational policies.
  • Meets DCF timelines: connect with the family within 24 hours of referral; facilitate initial family meeting within 48 hours; complete assessment-based case plan within 20 days; and close case within 3-6 months.
  • Engages and partners with families to complete assessments and develop case plan goals and objectives.
  • Meets with the family in the home weekly, monitoring progress toward case plan goals; meets monthly with each child individually to assess safety.
  • Assesses each child's safety with every family contact and reports all suspected child abuse/neglect. Monitors progress and updates case plan using assessment tools.
  • Provides weekly home‑based family skills training covering Nutrition, Hygiene, Household & Time Management, Home & Child Safety, Basic Parenting, Transportation, Problem Solving, and Communication.
  • Works closely with the Family Support Worker to provide services for the family and make decisions about child well‑being.
  • Prioritizes identified problems, establishes goals and objectives, utilizes resources, and makes appropriate decisions for the family.
  • Completes and analyzes initial and ongoing family assessments covering safety, stability, mental health, substance abuse, education, emotional, social, physical well‑being, parenting and parent‑child relationships.
  • Uses assessments to develop the case plan in partnership with the Family Support Worker/Family Mentor and children/family; makes community referrals as necessary.
  • Completes and submits timely court reports; appears and testifies in court as required.
  • Collects drug screens as necessary.
  • Documents case information (court reports, emails, activities) weekly.
  • Responds promptly to family crisis situations, documents incidents, and is available 24/7 to clients.
  • May need to drive regularly to various DCCCA locations and other business locations.
  • Promotes teamwork in all activities.
  • Fosters and maintains professional relationships with families, community partners and peers.
  • Participates in professional development that is relevant.
Education, Experience, and Licensure
  • Bachelor's degree in human services is required.
  • Prior work in behavioral health or children/family services is required; personal experience with addictions treatment is preferred.
  • Must possess strong verbal and written communication skills.
  • Demonstrates flexibility and adaptability; able to shift styles to fit the needs of a wide range of cultures, people, and organizations.
  • Solid commitment to the principles of support and advocacy for foster and adoptive families.
  • Must possess a valid state Driver's License, in-force automobile liability insurance, and maintain an MVR that meets DCCCA's insurability standard as defined by DCCCA policy. Availability of reliable transportation is required.
Knowledge, Skills, and Abilities
  • Overall computer knowledge and proficiency with computerized database programs, spreadsheet programs, word processing programs.
  • Ability to efficiently operate office equipment such as e‑mail systems, calculators, copiers, fax machines, telephones, and voicemail systems.

Effective listening, oral, and written communication skills, including face‑to‑face, on‑site, in‑person discussions.
