Crisis Case Manager

Overview

Crisis Case Manager position at Sumner Mental Health Center, a Certified Community Behavioral Health Clinic (CCBHC) and Community Mental Health Center (CMHC) located in Wellington, Kansas. The Crisis Case Manager will provide crisis services as part of the Crisis Services team, including assessment, intervention, care coordination with resources, and follow-up to ensure needed services are in place. After hours on-call is limited to a few times per year, with hours generally the regular work week from Monday through Friday, 8 AM to 5 PM.

• Provide crisis services to adults, youth, or children, including assessment, intervention, and care coordination with resources.
• Conduct follow-up to ensure needed services are in place and effective.

• Minimum of a Bachelor’s degree with experience or an Associate’s Degree; preferably with prior experience in providing crisis services.
• Experience with electronic medical records (EMR), documenting, and strong communication skills.

  Regular work week Monday through Friday, 8 AM to 5 PM; limited after-hours on-call a few times per year
• Background checks required;
  Kansas driver’s license and motor vehicle insurance required for use of fleet vehicles
