Mental Health, Clinical Social Worker
The Behavioral Health Social Worker serves as an expert resource to the Case Management Team, physicians, inter-disciplinary team members, patients’ and patients’ families in a collaborative process evidenced by assessing, planning, and facilitation of options and services to meet individuals’ complex biomedical and psychosocial needs. Also provides support to in-patient nursing staff for appropriate discharge planning and provides quality oversight of the work of non-licensed Social Work staff.
QUALIFICATIONS- Master’s Degree in Social Work required, or MFT or LMFT; with current and active license.
- Five years preferred, experience in social services support to clients, preferably in a behavioral health care setting, self-motivated with effective critical thinking skills, ability to work collaboratively as part of the clinical team and in-depth knowledge of government entitlement programs and community resources, knowledge of Behavioral Health Issues and comprehensive collaboration experience in a team environment.
- Current BLS for healthcare providers (American Heart Association) required.
- CPI certificate within 30 days of hire required.
- Take a lead role in interviewing, assessing individuals with complex behavioral health needs.
- Develops integrated/individualized behavioral health treatment goals as part of a comprehensive care plan.
- May conduct group or individual therapy depending on program needs identified by BH unit leadership.
- Provides appropriate and timely documentation regarding complex biomedical and psychosocial issues.
- Manage Behavioral Health referrals and provide navigation to patients and physicians to access appropriate behavioral healthcare services.
- Collaborate in discharge planning for a patient’s safe transition to home or to another level of care in collaboration with the patient, the patients’ family and the health care delivery team.
- Collaborates with the patients’, families and Case Management Team, Coordinators, Providers and Physicians in the implementation of a comprehensive plan of care.
- Develops and implements efficient processes for referring patients and families to Federal, State, and County assistance programs.
- Provides ongoing education to Medical Services Department and Physicians on locally available community resources and develops and maintains effective relationships with staff in key community resource agencies.
- Actively participates in team meetings and selected case conferences to provide assistance in identifying and resolving biomedical and psychosocial needs of patients and families.
- All other duties as assigned. May be required to cross train and work within scope of service in other areas within the Case Management department.
Salary: $30.46 - $38.07 per hour
The schedule for this position is Tuesday - Saturday. Day shift.

Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for CHLB roles, including but not limited to shift differential and other special pay practices, etc. The posted compensation for the position is a reasonable estimate that extends from the lowest to the highest pay that CHLB in good faith believes it might pay for this particular job, based on the circumstances at the time of posting.
