Job Title:

Social Service Coordinator

Job Responsibilities:

• Plan, develop, organize, implement, evaluate and direct the social service programs of the community in accordance with current state and federal regulations as well as professional standards

• Coordinate all resident discharges (including DME, following therapy services). Initiate and drive discharge planning starting on admission,

• Develop and implement social care plans and resident assessments ensuring that social services components of the MDS are completed and signed on a timely basis

• Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment

• Interview resident/families as necessary

• Assist in arranging transportation to other facilities

• Refer resident/families to appropriate social service agencies when the facility does not provide the services or needs of the resident.

• Provide information to residents/families as to Medicare/Medicaid, and other financial assistance programs available to the residents.

• Monitor the facility's, and survey reports relative to social services and make recommendations to correct identified problem areas.

• Attend all resident care plans

• Ensure the completion of PASRR screening, initiates the CARE assessment process with KDADS and the Area Agency on Aging.

• Schedule and perform admission assessments and annual assessments, ensure that care plans and CAA documentation are completed regarding residents' social history, cognition, trauma informed care, self-determinants of health and signs of depression, and other social service needs.

• Assist with determining which services are eligible/covered under residents' HMO insurance or Medicare; assists with Part B authorizations, LTC insurance updates, and applications for Medicaid. Assist as needed in completing documentation for financial resources such as LTC insurance, Medicaid, Veteran's benefits, etc.

• Assess the social and emotional factors within 5 days of admission in order to estimate the patient's and family's capacity and potential to cope with the problems of daily living and with the terminal illness.

• Help the patient and family to understand, accept and follow medical recommendations and provide services planned to assist the patient and family in achieving the optimum social and health adjustment within their capacity.

• Utilize all available resources, such as family and community agencies, to assist the patient and family to live better within the limitations of the illness.

• Observe, record, and report social and emotional changes.

• Document all services in the patient's clinical record.

• Participate as a member of the Interdisciplinary team and actively participates in the development and review of the plan of care for assigned patients.

• Participate in ongoing educational programs.

• Participate on committees and performs other duties and special projects as assigned.

• Complete all required documentation in a timely manner

• Explain hospice services and Medicare benefits to patients and families; obtains Informed Consent and Election of Benefits documents as requested.
