The Medication Manager will assess and diagnose psychiatric disorders for new patients and prescribe/monitor appropriate medication. Manage services to individual clients to promote optimal health. Is responsible, in collaboration with the patient, to establish treatment goals and prescribe medication regimen. In addition, the Medication Manager will monitor medication prescribed for efficacy, side effects and patient satisfaction as well as provide health education and instruction.
ESSENTIALPOSITION RESPONSIBILITIES
- 1. Provides services in order to maintain required productivity/billing standard of 70% of hours worked per year.
- 2. Meets deadlines and ensures accuracy of various report/paperwork, mileage records, and electronic timesheets.
- 3. Maintains accurate and timely documentation of service provision. Completes progress notes in a manner that individualizes each note, reflecting appropriate interventions and progress towards goals.
- 4. Submits required progress notes/billing information in a timely manner as per agency, Medicaid, and insurer guidelines and contracts.
- 5. Conducts comprehensive assessment and diagnosis on all clients referred for services.
- 6. Communicates with supervising physician on medication regimes as needed, ensuring adherence to established protocols.
- 7. Under established protocols, prescribes medications and provides medication checks; monitors and regulates various dosages of medications for each client. Medication management to include: review of laboratory reports, consultation with patient about the medical treatment plan, documentation in electronic medical record, answer questions on open and/or potential clients on treatment available.
- 8. Administers injections to clients according to physician orders.
- 9. Provides staff and family medication consultation around medication and other clinical concerns.
- 10. Evaluates clients for side effects and efficacy of psychotropic medications and reports problems to the client’s physician.
- 11. Triages client mental health or medication problems and reports to the physician.
- 12. Coordinates client appointments for laboratory tests.
- 13. Uses appropriate resources available within the community to optimize service to clients. Provides consultation to the treatment team.
- 14. Completes informed consent forms with patient and explains the risks and benefits of the treatment selected.
- 15. Performs quality improvement activities within area of responsibility.
- 16. Participates in scheduled meetings and supervision with other providers.
- 1. Maintains acceptable overall attendance record, including department staff meetings, agency meetings, and training as required. Ensures appropriate notification to supervisor for absences and ensures that work is covered. Flexible work schedule when needed.
- 2. Exhibits appropriate level of technical knowledge for the position.
- 3. Produces quantity of work necessary to meet job requirements.
- 4. Works well with a team, keeps others informed of information needed. Treats others with respect, maintaining a spirit of cooperation.

- 8. Performs work conscientiously with a high degree of accuracy.
The Medication Manager must have a Master’s degree (or above) and be currently licensed as an APRN with a specialization in Psychiatric-Mental Health Nursing. Board certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP) is highly preferred.…
