Job Overview
The Registered Nurse provides skilled nursing services to home care clients according to physician‑prescribed plans of care, manages the care delivery, coordinates with the healthcare team, and ensures quality outcomes through the nursing process.
TitleRegistered Nurse (RN) – Bronson At Home – Full‑time Days
LocationBAH Bronson at Home
Responsibilities- Collect comprehensive patient data and assess physical and developmental needs.
- Analyze assessment data to identify patient problems, confirm them with patient or caregiver, and develop individualized care plans that incorporate patient/family input.
- Plan and coordinate interventions, including medication administration, evidence‑based practices, and patient education.
- Implement and evaluate the effectiveness of nursing interventions, documenting changes and outcomes.
- Coordinate discharge planning and continuity of care.
- Collaborate with other health professionals to execute the patient’s plan of care.
- Delegate tasks to support staff within scope of practice.
- Maintain accurate documentation for each visit, including vital signs and patient response.
- Drive own vehicle on a regularly scheduled basis, maintain a valid driver's license, prove insurance coverage, and pass an MVR check.
- Communicate with team members and families verbally, in writing, and electronically.
- Diploma, Associate’s, or Bachelor’s degree in Nursing.
- Licensed RN in good standing with the State of Michigan.
- Current driver’s license in Michigan and proof of valid auto insurance.
- BLS certification.
- Minimum 2 years of continuous acute care experience (preferred).
- Systematically evaluate the quality and effectiveness of nursing practice.
- Participate in quality improvement activities and use results to initiate practice changes.
- Maintain professional records that document competency and ongoing education.
- Provide age‑appropriate, culturally, and ethnically sensitive care.
- Collaborate with multidisciplinary teams and contribute to the professional development of peers.
- Integrate ethical principles, patient autonomy, dignity, confidentiality and rights into all aspects of care.
- Utilize research and evidence‑based practice to improve patient health and organizational quality.
- Participate in research activities commensurate with education level.
- Assume leadership in professional practice settings and committees.
- Attend at least 50% of unit staff meetings and promote a healthy work environment.

- Full time – 40 hours per week
