Reporting to the Managing Director – Hospice, the RNCase Manager, provides skilled nursing care to hospice patients in their homes while coordinating individualized care plans with patients, families, and the interdisciplinary team (IDT). You'll serve as a trusted advocate, ensuring compassionate, high-quality care throughout the hospice journey.

What You’ll Do

Key Responsibilities
  • Perform comprehensive patient assessments and develop individualized care plans
  • Deliver skilled nursing care inpatients homes in accordance with hospice standards
  • Coordinate care with patients,families, physicians, and the IDT
  • Supervise Hospice Aides and LVNs toensure continuity of care
  • Maintain accurate, compliant clinical documentation
  • Complete patient admissions and participate in PRN on-call rotation
  • Manage patient schedules and prioritize changing care needs
  • Promote infection control, patient advocacy, and agency values
  • Assist with training and mentoring new clinical staff as needed
  • Commit to ongoing professional growth and clinical excellence through continuous education
What We Are Looking For Qualifications
  • Graduate of an accredited Nursing program (Diploma, ADN, or BSN)
  • Active Texas RN license required;
    Current CPR certification
  • 1–3 years of clinical nursing experience required;
    Hospice, ER, or OR experience preferred
  • Reliable transportation, valid

    Texas driver's license, and current auto insurance
  • Must pass background check and provide required TB and Hepatitis documentation
Additional Requirements
  • Ability to stand, walk, safely move patients, lift up to 50 lbs. and travel locally
  • Ability to work effectively inhigh-pressure, fast-paced environments
  • Strong clinical judgment and quick decision-making skills
  • Ability to meet the individualized psychosocial needs of patients and families
