position summary

cardinal hospice is seeking a highly organized, compassionate, and dependable licensed practical nurse (lpn) – part-time intake coordinator to join our team. This position plays an important role in coordinating hospice admissions, scheduling patient visits, maintaining accurate records, and supporting the day-to-day administrative operations of the hospice team.

the ideal candidate will have strong communication and organizational skills, excellent attention to detail, and the ability to effectively coordinate multiple priorities while providing compassionate service to patients, families, referral sources, and the hospice care team.

this position offers a great opportunity for an experienced lpn seeking a part-time schedule with consistent hours and meaningful work in hospice care.

essential responsibilities

hospice intake & admissions

  • coordinate and assist with the hospice intake and admission process.
  • receive and process referrals from hospitals, physicians, facilities, agencies, and other referral sources.
  • obtain and enter accurate patient demographic, clinical, and insurance information.
  • assist with coordinating admission visits and ensuring timely communication with the hospice team.
  • maintain accurate and complete admission documentation.
  • coordinate communication between referral sources, patients, families, and hospice staff.

scheduling & visit coordination

  • schedule and coordinate hospice patient visits based on patient needs and the plan of care.
  • assist with coordinating nursing, aide, social work, and other hospice visits as appropriate.
  • communicate schedule changes and updates to appropriate team members.
  • help ensure timely coverage of patient visits and continuity of care.
  • respond professionally to scheduling questions and requests from patients, families, and staff.

data entry & records

  • enter and maintain accurate patient information within hospice systems.
  • process and maintain documentation related to admissions and discharges.
  • ensure records are accurate, complete, and entered in a timely manner.
  • maintain confidentiality of patient and organizational information.
  • assist with data tracking and reporting as needed.

discharges & administrative support

  • assist with coordinating hospice discharges and completing required documentation.
  • communicate discharge information to appropriate members of the hospice team and referral sources.
  • assist with various administrative functions supporting hospice operations.
  • provide support with billing applications and related processes as needed.
  • assist with other administrative and clinical coordination duties within the scope of the lpn role.

  • current and active licensed practical nurse (lpn) license in the applicable state.
  • previous hospice, home health, healthcare, intake, scheduling, or case coordination experience preferred.
  • strong organizational and time-management skills.
  • excellent written and verbal communication skills.
  • strong attention to detail and accuracy with data entry and documentation.
  • ability to maintain confidentiality and handle sensitive patient information appropriately.
  • comfortable working independently while collaborating effectively with a multidisciplinary team.
  • proficiency with electronic medical records and computer systems preferred.
  • ability to prioritize multiple responsibilities in a fast-paced healthcare environment.
