The Clinical Coordinator RN is responsible for providing clinical expertise and serving as a liaison within the organization to ensure effective patient care delivery. This role is ideal for experienced nurses looking to make a significant impact in a dynamic healthcare environment.

Full Time, Day shift
- Receive, evaluate, accept, and execute the initial patient intake process.
- Review patient chart for admission criteria and assess needs.
- Assure timely communication with company personnel and outside providers.
- Establish and maintain positive clinical rapport with patients and families.
- Communicate coordination of care with alternate providers and document communication.
- Collaborate with agencies delivering services on behalf of Optum Infusion Pharmacy.
- Utilize Electronic Medical Record (EMR) systems to communicate patient status.
- Maintain current nursing licensure in all states serviced.
- Assist in securing and maintaining necessary licensures and accreditations.
- Comply with State and Federal regulations and standards of practice.
- Participate in staff meetings with nursing and pharmacy.
- Maintain all mandatory education required upon hire and annually.
- Represent the company in clinical interactions as requested.
- Provide education regarding Optum Infusion Pharmacy services.
- Function as a clinical educator and resource.
- Represent self in a professional and ethical manner.
- Participate in cross-training of various clinical functions.
- Perform other duties as requested.
- Current, unrestricted Compact RN license required.
- 2+ years of experience in Infusion Therapy and Home Health.
- One of the following licenses required:
- RN (Registered Nurse) - State Licensure required
- Willing and/or able to obtain additional RN licensures in specified states.
