After hours Coordinator
The After hours Coordinator will efficiently coordinate direct services to children after hours, ensuring the following functions are completed efficiently; communications, placements, transportation needs, ensuring the needs of the youth's needs are met at night and provide crisis support.
- Manage afternoon and even huddle to ensure smooth transition of children from 1st to 2nd shift, and 2nd to 3rd shift.
- Provides crisis management, de-escalation, parenting skills, and behavior management via phone.
- Assesses safety for children and adults, including Saint Francis Ministries staff.
- Develops short term safety plans to re-solve crisis situations.
- Informs Leadership immediately of Significant Events.
- Works with internal and external partners to access supports to the child/family during a time of crisis.
- Coordinates transportation arrangements for multiple youth in a timely and efficient manner.
- Works in conjunction with Foster Care Homes Workers to locate out of home placements, planned and/or emergency, referred to the program by DCF.
- Notifies after hours Support Worker when appropriate after hours placement is found.
- Ensures all children being transported to placement have blue book, medications, and belongings.
- Documents information and completes all necessary Saint Francis Ministries forms according to timelines.
- Frequent communication/coordination with After Hours Supervisor, After Hours Support Workers and other After hours Coordinators during crisis situations.
- Tracks on call situations handled by local offices.
- Helps to manage on youth care support worker schedule in a fair and efficient manner.
- Reports critical incidents to DCF verbally within 12 hours, written within 24 hours.
- Implements Saint Francis Ministries policies and follows directives as required. Follows and adheres to all pertinent Saint Francis Ministries Standard Operating Procedures (SOP's), rules, personnel policies, and procedures; related accreditation and licensure standards; and federal, state, and local rules, statutes, regulation, and contractual terms.
- Is knowledgeable of and follows all safety procedures.
- Reports unusual incidents through appropriate Risk Management, clinical and safety channels.
- Ensures clients' rights are protected.
- Prefer Bachelor's degree in Human Services field from an accredited university preferably in the Human Services field.
- Prefer two years' experience working children and families.
- Must be 21 years of age.
- Must pass a drug screen, MVR, KBI, Child Abuse and Neglect Central Registry Clearance check and an Adult Registry check.
- FBI Fingerprint check
- Lifting requirement of 50 pounds.
- Must have valid driver's license, acceptable driving record and reliable transportation.
