2nd Shift Family Support Worker
The 2nd Shift Family Support Worker provides direct crisis management services including, but not limited to, in-home, parenting skills, life skills, child development and behavior management, to assist children and families in reaching permanency.
Responsibilities include:
- Providing direct in-home services including, but not limited to, crisis management, de-escalation, parenting skills, and behavior management.
- Supporting case plan goals through teaching, role modeling, and demonstration.
- Developing short term safety plans to resolve crisis situations.
- Working with community partners to access supports for children and families during a time of crisis.
- Frequent communication/coordination with on call screener during crisis situations.
- Supervising and transporting children and families, as needed.
- Documenting information and completing all necessary Saint Francis Ministries forms according to timelines.
- Supporting resource families to maintain placements and help children work toward permanency.
- Monitoring and supervising family interactions.
- Providing clerical support to case teams as needed.
- Assisting with filing or closing out files as need or requested by Supervisor.
- Reporting critical incidents to DCF verbally within 12 hours, written within 24 hours.
- Implementing Saint Francis Ministries policies and following directives as required. Following and adhering to all pertinent Saint Francis Ministries Standard Operating Procedures (SOP's), rules, personnel policies, and procedures; related accreditation and licensure standards; and federal, state and local rules.
- Ensuring clients' rights are protected.
- Reporting unusual incidents through appropriate Risk Management, clinical and safety channels.
- Being knowledgeable of and following all safety procedures.
Requirements include:
- High School diploma or equivalent.
- Prefer 2 years' experience working with children and families, or a college degree in human services.
- Must be 21 years of age.
- Must pass a drug screen, MVR, KBI, Child Abuse and Neglect Central Registry Clearance check and an Adult Registry check.
- FBI Fingerprint check.
- Lifting requirement of 50 lbs.
- Must have valid driver's license, acceptable driving record and reliable transportation.
