Overview
Reentry Services Liaison - SHERIFF-KPERS is responsible for coordinating resources and collaborating with stakeholders to ensure the daily operation of Reentry Services. The role maintains a caseload of individuals needing services to reenter the community, working with detention vendors/volunteers inside the facility and with community partnerships and resources.
Responsibilities- Direct Client Service:
Respond to inmate communications in a timely manner; provide assistance in writing a resume, creating a Kansas Workforce Center profile, and conducting job searches; meet with clients to discuss individual plans, living arrangements, or barriers to reentry; assist with gaining government identification; identify and secure in-home services to maintain stabilization once the client returns home; familiarize clients with guidelines and procedures of reentry services.
- Treatment Planning:
Identify service needs and develop treatment or referral services plans with clients; perform functional assessment of client to determine strengths and abilities; contact service providers to request and arrange services; coordinate and monitor services rendered; provide informational packets to clients to help facilitate mental health appointments; confer with client caregivers, families, physicians, lawyers, and other professionals to secure and coordinate services;
provide information packets to maintain basic needs (housing, food, medical); provide outreach, screening, and engagement services to potential participants; respond to immediate needs of referrals; assist with Mental Health Discharge Planner for discharge and coordination of transition to the community; provide outreach to court-ordered consumers; provide Targeted Case Management (TCM) and CPST for assigned consumers to aid in achieving treatment goals;
provide Attendant Care (AC) for assigned consumers.
- Statistics and Documentation:
Prepare Reentry Plan with client and family participation as needed; collaborate on review, updates, and progress toward goals; complete notes for records and releases as required; provide monthly statistics and progress reports on active clients and court progress.
- Bachelor’s degree in a human services field, education, or related field. Degree may be substituted for experience (one year of experience per year of education).
- Valid Kansas driver’s license and current proof of automobile insurance; ability to safely operate a motor vehicle and meet driver qualification standards per Sedgwick County policy.
- Pass background check and motor vehicle screens; complete orientations provided by Sedgwick County and Sheriff’s Office.
- One year of experience in a Community Mental Health Center.
