Family Mentor

Essential Functions

• Provides for the safety, stability and well-being of children and families during the referral period.
• Delivers culturally competent services using a strengths-based approach to children and families consistent with the evidence-based practice models.
• Meets DCF timelines to connect with the family and facilitate the initial family meeting.
• Engages and works in partnership with the case manager and family to complete assessments.
• Responds to calls and client emergencies as needed, including being on‑call and working some weekends and public holidays as necessary.
• Meets with clients at least once a week to offer one‑on‑one support.
• Develops recovery plans, coping methods and rehabilitation services with the client.
• Facilitates treatment assessments and referrals and supports clients in arranging for medical appointments, counseling, support groups and other resources as needed.
• Monitors the client’s behavior, schedule, medication and activities and reports any unusual behavior, conflicts or recovery deviations to the relevant professionals.
• Provides transportation and resources and coordinates community support services as needed.
• Job may require the employee to regularly drive to various DCCCA locations and other business locations throughout the day or night as required.
• Other duties as assigned.
