Job Title
Job Objective:
Work with individuals and families in a Transitional Housing/ Rapid Rehousing Program on three basic outcomes:
Daily Living, Financial Stability and Permanent Housing.
Conduct an initial assessment/history of the participant
Prepare a case history, and develop, with the participant, a housing plan designed to end their homelessness
Assist the participant in the development and implementation of a housing plan and monitor/document their progress toward the completion of the plan
Provide linkages/referrals/transportation to appropriate agencies and function as an advocate for the participant in the community as appropriate
Provide Case Management in a participant's home, in the community and at the facility.
Complete daily, weekly, monthly paperwork (Case Management meeting numbers, documentation, financial reports, check requests, beginning/end information in HMIS, etc.) in a timely and accurate manner
Participate in the development of a discharge plan to facilitate the participants' obtaining/maintaining permanent housing
Gather statistical information as required by funding bodies
Attend community networking meetings to enhance the service delivery system for the clientele
Provide community education regarding homelessness and The Salvation Army by conducting tours, participating in workshops/in services, speaking to community groups, etc.
Participate in agency and/or professional workshops/seminars to increase professional knowledge
Represent the agency/program in the community as assigned
Provide On-Call coverage as scheduled
As part of The Salvation Army comply with all confidentiality laws that apply to social welfare
Other duties as assigned by the Director
Minimum QualificationsEducation:
Bachelor degree in Social Work or related field
Experience:
Prior experience in Social Services or residential setting; knowledge of homelessness and ability to work with and supervise a diverse clientele; knowledge of counseling/crisis intervention techniques
Certifications/Licenses:

Effective communication skills (written and oral)
Ability to prioritize job responsibilities
Knowledge of community resources
Ability to network
Supportive of administrative and program objectives
Must have a valid driver's license, maintain insurance, meet and maintain "qualified driver" status
Ability to operate basic office equipment including telephone, personal computer
Supervisory ResponsibilityCase Manager will supervise Intake Workers while on-call
Physical RequirementsSpeaking, hearing and vision ability; manual dexterity; walking; sitting and standing for extended periods of time; lifting, pulling and pushing materials up to 25 pounds;

May be required to attend conferences
DrivingMust possess a valid driver's license issued by the state of residence. Must be approved through The Salvation Army Fleet Safety Program to drive either a Salvation Army or personal vehicle on Salvation Army business
Working ConditionsResidential setting – office space is separate from participant living area
Hours:
Monday
-Friday, 9a-5 p. Flexibility in hours possible to meet participant time constraints
After hours, weekend and holiday hours while On-Call
Extended hours during special events (Young Lawyer's Party, Toy Distribution, etc.)
