Connections Case Manager

Ember Hope is excited to welcome new Case Managers to our Child and Family Wellbeing team. This role is designed for individuals who are passionate about walking alongside children, youth, young adults, and families as they navigate important moments of change, healing, and growth.

In Ember Hope’s 90-day onboarding model, new Case Managers will receive focused training on the essential functions of case management, including engagement with children and families, assessment and case planning, documentation, court and stakeholder collaboration, resource connection, safety and permanency planning, and trauma-sensitive service delivery. After successful completion of onboarding, Case Managers will be placed in one of our Child and Family Wellbeing departments based on program needs, strengths, and fit:
Adoption, Crossover, Independent Living, Reintegration, or Aftercare.

What You’ll Do
  • Build meaningful, professional relationships with children, youth, young adults, and families.
  • Provide trauma-sensitive case management services that promote safety, stability, resilience, and well-being.
  • Partner with families to identify strengths, needs, barriers, and goals.
  • Support case planning, resource connection, service coordination, and progress toward permanency or successful transition.
  • Visit with children and families regularly to assess needs, support progress, and ensure safety concerns are addressed.
  • Collaborate with courts, DCF, providers, schools, community partners, foster parents, relatives, and other stakeholders.
  • Prepare and submit timely documentation, reports, assessments, and case updates according to agency and program requirements.
  • Advocate for children and families while helping them navigate complex systems with dignity and support.
  • Attend meetings, court hearings, staffing discussions, and team consultations as needed.
  • Maintain compliance with company policy, practice guidelines, training expectations, and regulatory requirements.
