2ND Shift

Hours:

10:00 AM to 6:00 PM

The Hope Unified for Belonging Specialist (HUB) provides intensive case management and support to youth in foster care who are without placement or are experiencing placement instability. This highly specialized staff works with a small caseload of youth, providing direct support, advocacy, and crisis intervention to help them achieve stable, long-term placement solutions. The HUB also works closely with families, caregivers, and other service providers to ensure that the youth's needs are met and that they are able to thrive in their placements.

• 1. Provide direct case support services to a small caseload of youth (maximum of 5 youth) who are without placement or experiencing placement instability.
• 2. Conduct weekly visits with each youth, providing support, guidance, and advocacy.
• 3. Develop and implement individualized plans of care that address the youth's unique needs and goals.
• 4. Facilitate access to mental and physical health services, ensuring the youth's well-being is prioritized.
• 5. Conduct thorough family finding activities to re-engage kinship connections and explore potential long-term placement options within the family network.
• 6. Provide direct support to youth attending day service programs, particularly during times of emotional distress.
• 7. Collaborate with the youth's treatment team, including therapists, psychiatrists, and other service providers, to ensure that their needs are met.
• 8. Advocate for the youth's needs in various settings, including schools, courts, and social service agencies.
• 9. Intervening and assisting when a youth gets expelled from school or a placement.
• 10. Assisting with encouraging the child to go to and engage in their placement.
• 11. Encouraging the child to attend and participate in appointments (medical, therapeutic, etc.).
• 12. Preparing the child for transitions to other placements, court appearances, and other significant events.
• 13. Daily Check-Ins:
  Conducting daily check-ins with staff at whatever night-to-night or day program the youth is placed in to ensure that all individualized needs are being met.
• 14. Provide crisis intervention and support to youth and families as needed.
• 15. Maintain accurate and timely records of all services provided.
• 16. Participate in regular supervision and team meetings.
• 17. Adhere to all agency policies and procedures
• 18. Participate in on-call rotation.

• 30% Crisis Intervention

• Education:
  
  HS Diploma or GED, bachelors preferred
• Experience:
  
  5 years of experience working with youth in foster care or a related field.
• Training:
  Must have completed or be willing to complete Trust-Based Relational Intervention (TBRI) and Safe Crisis Management (SCM) training .
• 21 years of age. Must pass a background che ck, fingerprinting, and physical and drug screening.
