RN/LPN Responsibilities
• Provide skilled in-home nursing care for low- to high-acuity patients
• Complete patient assessments and follow individualized care plans
• Administer medications and treatments as prescribed
• Collaborate with families and interdisciplinary team members
• Accurately document care and report changes to the RN Clinical Supervisor/Site Director
• Safely assist with lifting, positioning, and transferring patients
Qualifications
• Active Arizona RN or LPN license (or Compact License)
• Valid Arizona Driver’s License
• Current BLS Certification (online-only courses not accepted)
• Current TB test or chest X-ray within the past 12 months
• Level One Fingerprint Clearance Card
• Article 9 Training (assistance available)
• Strong communication, time management, and problem-solving skills
