Memory Care Coordinator CDP
About the Role
At Meadow Falls of Wickliffe, we are committed to providing the highest quality care and support to our residents with memory impairments. As a Memory Care Coordinator CDP, you will play a vital role in ensuring our residents receive exceptional care and services tailored to their unique needs. Located in Wickliffe, OH, our community is designed to provide a safe, engaging, and supportive environment for individuals with Alzheimer's disease, dementia, and other memory impairments.
Responsibilities- Develop and maintain relationships with residents, families, and care providers to ensure seamless communication and coordination of care
- Conduct memory care assessments and develop individualized care plans to meet the unique needs of each resident
- Collaborate with interdisciplinary team members to deliver comprehensive care and services, including medication management, behavioral interventions, and environmental modifications
- Implement and monitor treatments and interventions to ensure optimal outcomes and quality of life for residents
- Provide education and support to families and caregivers on memory care best practices and available resources
- Participate in resident care conferences and meetings to review progress and identify areas for improvement
Maintain accurate and thorough records of resident care, progress, and treatment plans
- CDP certification or willingness to obtain within 6 months of hire
- Minimum 0 years of experience in memory care or gerontology
- High school diploma or equivalent required; degree in healthcare-related field or business administration preferred
- Ability to communicate effectively with residents, families, and care providers in a supportive and compassionate manner
- Strong organizational, problem-solving, and time management skills to manage diverse responsibilities and priorities
- Ability to work effectively in a fast-paced environment with multiple demands and deadlines
