Position OverviewAs a Medication Management LPN (Licensed Practical Nurse), you will play a crucial role in ensuring the safe and effective administration of medications for our clients. You will conduct in‑home visits, educate patients and caregivers, and collaborate with healthcare professionals to enhance patient well‑being.
Key Responsibilities- Conduct in‑home assessments to review and manage client medications.
- Educate clients and caregivers on proper medication usage
, potential side effects, and adherence. - Set up and organize medications, including pre‑filling medication planners when necessary.
- Collaborate with physicians, pharmacists, and the care team to address medication‑related concerns.
- Monitor clients for adverse reactions and report any significant health changes.

- Ensure compliance with healthcare regulations and company policies.
- Conduct thorough health assessments
