Job Summary: The Geisinger Skilled Nursing Program is seeking an experienced Nurse Practitioner or Physician Assistant to 'float' to various skilled nursing facilities in northeast Pennsylvania.

This position is as a Geisinger provider in a non-Geisinger facility and requires an experienced advanced practitioner (AP), prior experience working in a skilled nursing facility, family medicine, internal medicine or ICU. The AP provides direct (hands‑on) care in skilled nursing facilities with a goal to improve quality and prevent unnecessary re‑admissions to the acute care setting.

This position has a $10,400 float differential and will travel to these locations, but are not limited to:

• Mountain Top Rehabilitation & Healthcare Center, Mountain Top
• Birchwood Rehabilitation and Healthcare Center, Nanticoke
• Lakewood Rehabilitation & Healthcare Center, Nanticoke
• The Meadows, Dallas
• Embassy of Wyoming Valley, Wilkes Barre
• Embassy of Scranton, Scranton
• Oak Ridge Rehabilitation & Healthcare Center, Scranton

• Continuing Medical Education – 10 days and $3,000 annually

• Review reports
• Communicate with SNF staff for patient needs
• Treat patients for acute needs, admissions, discharges, etc.
• Manage extremely complex patients, see every day or at least once a week
• Approximately 12 patients a day
• Length of time per patient and number of patients is per need

Education: Master's Degree-Physician Assistant (Required)

Experience:

Certification(s) and License(s): Basic Life Support Certification - Default Issuing Body, Certified Nurse Practitioner - American Academy of Nurse Practitioners, Licensed Registered Nurse (Pennsylvania) -  of Pennsylvania
