Licensed Practical Nurse – Neurology

Role Overview: The Licensed Practical Nurse (LPN) serves as a patient navigator, guiding incoming calls, facilitating patient onboarding, and providing education to promote understanding and ongoing care. This full-time position involves clinical support, collaboration with healthcare providers, and assisting in diagnosis and treatment processes.

Key Responsibilities:

• Utilize clinical knowledge to evaluate patient needs primarily via phone or documentation review.
• Coordinate and communicate with patients and providers regarding care plans, appointments, and support services.
• Assist with patient activities such as support groups and educational programs.
• Develop and implement screening tools and navigational procedures for efficient patient flow.
• Conduct medical record reviews, provide nursing care, and support physician procedures.
• Maintain clinical supplies, ensure equipment functionality, and oversee exam room readiness.
• Investigate patient concerns related to access, communication, and billing.

Qualifications & Skills:

• Minimum of 2 years LPN work experience.
• Graduate from a specialty nursing training program.
• Strong communication, teamwork, and organizational skills.
• Ability to collaborate effectively within interdisciplinary teams.
