Role Overview: This position involves serving as an expanded nursing role within an interdisciplinary team to promote preventative care, conduct screenings, and coordinate disease management services. The nurse provides primary clinical support to providers, patients, and families, assisting in diagnosis and treatment planning. Key responsibilities include patient assessment, education, disease management, and collaboration with healthcare providers to ensure adherence to clinical guidelines.
The role also involves managing laboratory and diagnostic tests, administering medications and vaccines, and maintaining detailed documentation.
Key Duties:
- Assess healthcare, educational, and psychosocial needs to develop individualized care plans.
- Facilitate communication among patients, families, and healthcare teams to overcome barriers and optimize treatment.
- Educate patients and families on disease management, care coordination, and health strategies.
- Implement clinical interventions based on risk assessment and evidence-based guidelines.
- Coordinate diagnostic testing and monitor therapy adjustments in collaboration with providers.
- Document activities accurately in electronic health records and collect relevant data.
- Participate in ongoing professional development and incorporate current best practices.
- Support post-discharge transitions and contribute to program design and evaluation.
- Perform clinical procedures such as IV insertion, medication administration, and specimen collection, within scope of practice.
Qualifications & Skills: A minimum of 2 years of RN experience is required. Education includes a graduate from a specialty nursing training program, with a Bachelor’s degree preferred. Strong clinical, communication, and collaborative skills are essential for success in this role.
