Colonial Behavioral Health in Virginia seeks a Case Manager for the Adult Mental Health department to work with adults experiencing serious mental illness and/or substance use disorders. You will help clients identify goals, navigate challenges, connect with resources, and access services to thrive.
This role emphasizes collaboration with families, maintaining confidential documentation, and coordinating supports to help individuals meet their needs. Work hours are Monday–Friday, 8:30 a.m.
