Williamsburg, VA
Coverage area: Williamsburg, Peninsula
Schedule: Full-Time;
Monday - Friday

• Providing 1:1 care for the client following the client’s plan of care at all times while maintaining the client’s dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client’s plan of care and providing assessments of results
• Accurately documenting nursing actions, communication with family, case manager, and physicians, and documenting progress and outcomes for established goals; informing physicians, case managers, and families of changes in the client’s medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to ensure adequate amounts are available
• Ensuring all patient needs are met in accordance with the physician’s instructions

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and state of residence
• Valid driver’s license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred

Salary Range: USD $25.00 - $38.00 / Hour
