Licensed Practical Nurse

Williamsburg Landing, a not-for-profit Life Plan Community in Williamsburg, VA, seeks a Licensed Practical Nurse to deliver resident-centered care in assisted living, memory support, and skilled nursing. The LPN will provide medication administration, treatments, and clinical monitoring while supporting wellness, dignity, and safety. Collaborating with an interdisciplinary team, the LPN will supervise CNAs, document care, and respond to changes in resident condition.

Responsibilities

• Provide direct nursing care to residents including medication administration, treatments, and monitoring of health status
• Assist with assessments, care planning, and documentation in accordance with state regulations and facility policies
• Collaborate with RNs, physicians, CNAs, therapists, and other team members to deliver coordinated, resident-centered care
• Respond to changes in resident condition, notify appropriate clinicians and families, and implement interventions
• Educate residents and families on care plans, treatments, and health maintenance
• Support a safe, clean, and dignified environment, following infection control and safety protocols
• Supervise and support CNAs and other support staff, ensuring quality and timely care
• Participate in shift reports, care conferences, and ongoing quality improvement initiatives
