In this hybrid position, the clinician will float to health centers to provide in clinic patient care within a designated region, cover telehealth sessions (working from a health center or administrative building), cover asynchronous e-visits, support health center teams by managing EHR Inbaskets and follow up, and assist as needed in the Care Coordination Team. The exact mix of these activities will depend on the needs of the organization.
The clinician will float to various health centers and will work under the supervision of the Center Manager and in collaboration with the health center care team, to provide direct patient care in accordance with PPSNE protocols both in person and on telehealth.
While working Care Coordination Team (CCT) sessions, under the supervision of the CCT Manager, the clinician is responsible for managing follow-up of abnormal results, signing off on lab reports and providing clinical and clinician support to the CCT. The clinician will communicate with patients regarding more complex care plans, such as beta quant HCG follow-up and urgent follow-up.
Education and Experience
- Graduation from a recognized Nurse Practitioner, Physician Assistant or Nurse Midwife training program or Medical School.
- Appropriate professional certification (ANCC, NCC, ACNM, NCCPA or MD)
- Active licensure in good standing (State Medical License for CT and/or RI, Controlled Substance Registration, DEA)
- Basic Life Support certification
- Experience in sexual and reproductive healthcare
- Primary care experience preferred
- Bilingual preferred (English-Spanish)

- Flexibility to pivot scheduled job role including covering unplanned health center absences.

- Ensures high quality direct patient interactions in person, on telehealth, and on asynchronous e-visits based on empathy for the sensitive issues facing our patients and an understanding of the diverse experiences of those seeking our services.
- Performs the physical exam, orders and/or performs appropriate laboratory testing and recognizes deviation from normal.
