About the Program:
Intensive In-Home Child and Adolescent Psychiatric Services (IICAPS) is a team-based, evidence-informed program that provides comprehensive community-based clinical, case management, and care coordination services for children and adolescents ages 3–18 who are at risk of hospitalization. The program focuses on stabilizing youth in their homes and communities, strengthening family relationships, and supporting long-term success through individualized, strength-based treatment.

About the Position:
Outreach Therapist This role offers a unique opportunity to provide meaningful, community-based clinical care while building strong skills in a supportive, team-oriented environment. As an IICAPS clinician, you will work directly with children, adolescents, and families in their homes, schools, and communities to support stabilization, strengthen relationships, and promote long-term success. Your work will be both independent and highly collaborative, allowing you to develop confidence in your clinical decision-making while receiving consistent supervision and support.

Position Highlights

  • Role is based out of the Manchester office and services provided throughout Greater Manchester
  • Additional coverage in Windham and Middlesex County may be required based on program needs
  • Caseloads are assigned geographically when possible, to support efficiency and work-life balance
  • Manage a structured caseload (typically 8 families) with 2–3 sessions per week per family
  • Provide services in home, school, and community settings, offering variety and flexibility in your day
  • Maintain a flexible schedule, with daytime and some evening availability based on family needs
  • Staff work a hybrid model from home or the office
  • Work as part of a collaborative 2-person clinical team
  • Receive weekly two-hour team supervision and ongoing clinical consultation. Supervision towards licensure is provided weekly and supportive team environment.
  • No on-call requirements on weekends or holidays

Duties & Responsibilities

  • Provides assessment and treatment interventions consistent with the program model.
  • Conducts clinical and risk assessments.
  • Collaborates with individuals to create individualized, person-centered treatment plans.
  • Demonstrates competency in age related or specialty issues and developmental needs for each population served.
  • Works with clients to develop appropriate discharge plans utilizing natural support.
  • Maintains collaborative relationships with family members and other service providers.
  • Provides crisis assessment and intervention to monitor status of clients in crisis.
  • May provide care in office, home, residential, and/or community-based settings based on program model and setting.
  • Responsible for maintaining client and staff records according to agency policy.
  • Provides coverage for team members during periods of absence.
  • Participates in weekly supervision, consultations, and meetings.
  • Transports clients using agency vehicles and/or personal vehicles based on program model and setting.
