SNF Social Worker - Travel

Minimum 1 year of experience working in a Skilled Nursing Facility

Temp Social Work is currently seeking an experienced social worker to fill an opening with a skilled nursing facility located in the Twin Cities. The ideal candidate should be able to perform the duties of a nursing home social worker.

Starting rate of $35/ hour (If you do not meet the criteria, the rate may differ)

Requirements of the Social Worker

• Bachelor's or Master's degree in Social Work from an accredited school
• Valid Social Work License in the state of Minnesota
• One year of experience working in a nursing home
• Knowledge of community based services
  - Preferred

Responsibilities of the Social Worker

• Conduct and document psychosocial assessments
• Complete the MDS, care plan and any other required paperwork
• Communicate with resident families as needed during the course of providing social services and discharge planning
