Director of Rehab; DOR In House SNF

Job Summary

Director of Rehabilitation (PT, PTA, OT, OTA, SLP) for an In-house Skilled Nursing Facility in Franklin, MN. This position is responsible for all aspects of therapy operations at the facility in accordance with established policies, procedures, and practices of the company under the direction of the Regional Director of Operations. Includes a $5,000 sign-on bonus.

• Oversee and manage all aspects of the therapy program in the assigned facility/facilities.
• Maintain knowledge of, and adherence to, regulatory requirements, company policies, procedures, processes and guidelines, job descriptions, certification requirements, HIPAA, confidentiality standards, and resident rights.
• Establish facility-specific procedures in collaboration with the facility interdisciplinary team, and train all therapy staff on established procedures.
• Ensure all therapists provide therapy services in accordance with regulatory requirements, established standards of practice, Company policies and procedures, therapy department procedures, and productivity standards.
• Ensure all therapists adhere to regulatory requirements and Company procedures regarding documentation and billing of therapy services.
• Communicate and interact professionally and respectfully with patients and family members in attending to and meeting their requests.
• Perform all other duties as assigned and as required to effectively discharge the responsibilities of the position and in the best interests of the organization.

• Degree in Physical Therapy, Occupational Therapy, or Speech-Language Pathology.
• Must have active licensure (PT, PTA, OT, OTA, or SLP) in the state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.
• Experience with PDPM and MDS management preferred.
