Occupational Therapist (OT) – Skilled Nursing Facility (SNF)

Setting: Skilled Nursing Facility (SNF)
Schedule: Full-Time
Compensation: $72,000–$110,000 annually, depending on experience and certifications

Position Summary

As an Occupational Therapist, you will provide skilled occupational therapy services to residents recovering from illness, injury, surgery, or age‑related functional decline. You will work closely with residents, families, and interdisciplinary team members to develop treatment plans that promote independence, safety, and participation in meaningful daily activities.

Responsibilities
  • Evaluate residents and develop individualized occupational therapy treatment plans.
  • Assess functional abilities related to activities of daily living (ADLs), mobility, cognition, upper extremity function, and safety awareness.
  • Implement evidence-based interventions designed to improve strength, coordination, endurance, balance, and independence.
  • Recommend adaptive equipment, environmental modifications, and compensatory strategies to enhance resident safety and function.
  • Educate residents, caregivers, and family members regarding treatment goals, home programs, and safety recommendations.
  • Maintain timely and accurate documentation in accordance with facility, state, federal, and payer requirements.
  • Collaborate with Physical Therapists, Speech‑Language Pathologists, nursing staff, physicians, and other interdisciplinary team members.
  • Participate in care conferences, discharge planning, and quality improvement initiatives.
  • Support departmental programs and contribute to a positive team-oriented culture.
Qualifications
  • Graduate of an Accreditation Council for Occupational Therapy Education (ACOTE)-accredited Occupational Therapy program.
  • Current Minnesota Occupational Therapist License issued by the Minnesota Board of Occupational Therapy Practice, or eligibility to obtain licensure prior to start date.
  • Current registration with the National Board for Certification in Occupational Therapy (NBCOT) preferred or eligible as required for licensure.
  • Skilled Nursing Facility, long‑term care, geriatric rehabilitation, or inpatient rehabilitation experience preferred but not required.
  • Strong communication, interpersonal, organizational, and documentation skills.
  • Ability to work effectively within an interdisciplinary healthcare team.
