JOB SUMMARY

The Regional System and Project Coordinator leads implementation of grant-funded Rural Health Transformation Project focused on Adolescent and Children’s Mental Health designed to strengthen early identification, referral coordination, family engagement, care coordination, continuity of care, and access to supports for children, youth, and families across the five-county region. This position serves as the primary liaison among schools, county agencies, public health, healthcare organizations, mental health providers, community organizations, and families to strengthen collaboration, increase awareness of available services, improve referral pathways, and support achievement of project goals and outcomes.

SPECIFIC

RESPONSIBILITIES
  • Coordinate implementation of Rural Health Transformation Project
    - Adolescent & Children’s Mental Health Initiative.
  • Monitor project timelines, deliverables, performance measures, and implementation activities to ensure achievement of project goals and objectives.
  • Maintain regular communication with schools, county agencies, healthcare organizations, mental health providers, and community partners to increase awareness of services, identify emerging needs, strengthen referral pathways, and improve access to supports.
  • Coordinate outreach, engagement, and collaborative planning activities that strengthen cross-sector partnerships, referral pathways, and coordinated systems of care.
  • Support implementation of family-centered planning approaches, including Wraparound, Family Group Decision Making (FGDM), and other evidence-informed strategies as appropriate.
  • Coordinate training, technical assistance, and capacity-building activities related to family engagement, care coordination, referral pathways, and systems collaboration.
  • Maintain project records and collaborate with project staff, evaluators, and partners to support data collection, program evaluation, quality improvement activities, and reporting requirements.
  • Prepare or assist with grant reports, implementation updates, and presentations and participate in collaborative, committee, and project meetings as assigned.
  • Represent PACT at community meetings, regional planning efforts, conferences, and partner events as assigned.
  • Other duties as assigned
QUALIFICATIONS AND PRIOR EXPERIENCE REQUIRED
  • Bachelor’s degree in education, social work, public health, human services, or a related field, or equivalent experience.
  • Experience in project coordinator, community engagement, behavioral health, education, prevention, family services, healthcare, or related fields preferred.
  • Strong communication, facilitation, relationship-building, and organizational skills.
  • Demonstrated ability to collaborate across agencies and build strong partnerships.
  • Understanding of behavioral health systems, education systems, county services, healthcare systems and community-based resources.
  • Experience working with diverse populations and commitment to culturally responsive practices.
  • Ability to analyze information, identify trends, and support quality improvement efforts
  • Strong problem-solving skills and the ability to facilitate collaborative solutions.
  • Ability to present information clearly and facilitate meetings, trainings, and group discussions.
  • Sensitivity and interest in supporting children, youth, families, and communities across varied backgrounds.
TRAVEL

There is significant travel within the five counties, the PACT service area, and occasional travel in the State. Valid driver's license, car insurance and vehicle required. Work hours may require late afternoon and evening meeting times and appointments.

HOURS WORKED AND SALARY

This is a full-time salaried position with comprehensive benefits. Salary will range from $55,359 annual, depending on experience, education and training.
