Child and Adolescent Caregiver

As a Child and Adolescent Caregiver at Sevita, you'll provide compassionate, person-centered support to children and teens in home and community settings. In this role, you'll assist with daily living activities, implement behavior and care plans, and promote safety, independence, and emotional wellbeing. You'll collaborate with clinical teams and families, document care, and support community and social activities. Responsibilities

• Provide direct care and supervision to children and adolescents in home and community-based settings
• Assist with daily living activities, hygiene, meals, and routines
• Implement individualized care and behavior support plans as directed by clinicians
• Promote safety, emotional wellbeing, and skill-building for independence
• Support school, recreational, and community activities and transportation as needed
• Monitor, document, and report progress, behaviors, and incidents accurately
• Communicate and collaborate with families, clinical staff, and supervisors
• Follow all health, safety, and confidentiality policies and procedures
• Participate in required training, professional development, and team meetings
