Compensation: $120,000 to $160,000+ Total Compensation (Base Salary + Productivity Incentive Program) + Mileage Reimbursement

Responsibilities- Provide on-site primary care and rounding services throughout senior living communities.
- Manage chronic conditions, preventative care, medication reconciliation, diagnostics, and treatment planning.
- Deliver wound care services and participate in advanced wound management initiatives.
- Coordinate care with physicians, nursing staff, therapists, caregivers, and families.
- Support continuity of care while helping reduce avoidable hospitalizations and emergency room visits.
- Build expertise in both primary care and wound care management.
- Active Minnesota Certified Nurse Practitioner (CNP) license.
- Experience in geriatrics, primary care, post-acute care, internal medicine, family medicine, or related settings preferred.
- Wound care certification welcomed but not required.
- Valid driver's license and ability to travel throughout the Twin Cities Metro Area.
- Passion for caring for aging adults and continuing to grow professionally.
Compensation: $105,000 to $120,000 annually + autonomy + long-term growth

Responsibilities- Provide on-site rounding, primary care, and advanced wound care services.
- Manage complex wounds including pressure ulcers, diabetic ulcers, and skin integrity issues.
- Drive chronic care management and preventative care strategies.
- Collaborate with physicians, nurses, and interdisciplinary care teams.
- Reduce ER visits and hospitalizations through proactive care delivery.
- Communicate with families, caregivers, and facility staff.
- Complete documentation, medication management, and lab coordination.
- Active Certified Nurse Practitioner (CNP) license in Minnesota (AGNP preferred).
- Experience in wound care, geriatrics, primary care, or post-acute care preferred.
- Valid driver’s license and ability to travel throughout the Twin Cities Metro.
- Strong ability to work independently and manage a mobile schedule.
- Passion for delivering high-quality, relationship-based senior care.
