Licensed Practical Nurse

A Licensed Practical Nurse (LPN) works collaboratively with internal and external providers to support integrated, client-centered care. This role assists in establishing care pathways for healthcare needs and helps clients access appropriate services and resources. The LPN provides in-person and phone support, problem-solving, education, patient advocacy, and follow-up care coordination. This position also serves as a liaison for psychiatric providers before appointments and during follow-up care.

This is a full-time position. Typical hours of work are Monday-Friday, 8am-5pm, or similar hours.

• Attend meetings and collaborate with internal and external providers to support the Integrated Care process.
• Must possess computer skills for accurate documentation, folder creation, and the ability to organize forms and other data on the computer.
• Demonstrate strong communication, assessment, and independent decision-making skills.
• Assist with the nursing phone line, process orders, facilitate medication reconciliation, coordinate client appointments, and perform other nursing-related duties.
• Provide services in accordance with the mission and values of Woodland Centers.

• Must have a valid MN Licensed Practical Nurse license.
• Valid driver’s license and pass a background check.

Salary From: $26.73 per hour

Expected hours: 40 per week
