RN CLINICAL MANAGER JOB DESCRIPTION

Job Summary

The RN Clinical Manager provides clinical leadership, oversight, and direct patient care for Home Health and Hospice services. This position is responsible for coordinating and managing patient care, supervising clinical staff, and ensuring compliance with agency policies, quality standards, and state and federal regulations. The RN Clinical Manager performs skilled nursing services in home, community, and on‑campus settings while supporting patients and families through all phases of care.

This role promotes high‑quality, compassionate, and patient‑centered care through effective case management, interdisciplinary collaboration, and clinical oversight.

Job Relationships
  • Reports to:

    Hospice Director
  • Supervisory Responsibilities:

    RN Case Managers, LPNs, Hospice Care Aides and Homemakers.
  • Internal Relationships:
    Other RN Case Managers, Scheduling Coordinator, Bethesda Social Service departments, Volunteer Coordinator, Housing Managers and Coordinators, Contracted Therapists and Medical Director.
  • External Contacts:
    Medical Director, Physicians, Pharmacists, Health Service personnel in surrounding area, Kandiyohi County Family Services Social Workers and Public Health Nurses, Nursing Home Social Workers, Hospital Discharge Planners, Insurance Company personnel, Patients and Family members, and other community providers.
Hours of Work

Monday through Friday; 8:00 a.m. to 4:30 p.m. Must be flexible for patient needs. Rotating on‑call duties, with evening and weekend visit expectations.

Dress Code

Street clothing or nursing uniform and identification badge.

Essential Requirements of Work
  • Education:
    • Graduate of an accredited school of nursing.
    • Current Minnesota registration as a Registered Nurse. Must maintain licensure.
    • Current CPR Certification or ability to immediately obtain certification.
  • Experience:
    • Strong proficiency in nursing skills.
    • Home Health and Hospice experience preferred.
    • Previous management experience preferred.
    • Basic Computer skills.
  • Personal:
    • Highly developed communication skills (oral and written).
    • Proven time management skills.
    • Understanding of basic quality principles.
    • Adaptable to various work settings.
    • Self‑motivated.
    • Attention to detail.
    • Problem solving skills.
    • Ability to direct and supervise.
    • Ability to ensure confidentiality.
    • Ability to learn from mistakes.
    • Provide a pleasant attitude and compassionate disposition towards staff, patients, and families.
    • Own vehicle, current valid driver’s license, and proof of current automobile insurance.
    • In accordance with CMS Vaccine Mandate, must provide proof of Covid‑19 Vaccine or submit documentation for exemption.
  • Job Knowledge:
    • Knowledge of nursing policies and procedures.
    • Knowledge of infection control practices.
    • Knowledge of State Federal regulatory requirements.
    • Knowledge of the Nurse Practice Act.
    • Ability to assess patient’s condition with knowledge to provide necessary action.
    • Supervisory knowledge and abilities.
    • Philosophical understanding of quality of life and quality process management.
    • Computer knowledge.
Physical Demands of the Job
  • Repeated bending, squatting, stooping, sitting, climbing stairs.
  • Occasional pushing/pulling movements of wheelchairs or patients weighing 100-250 pounds.
  • Frequent exposure to infection.

Provide and monitor quality care
  • Follow State and Federal regulations and agency policies and procedures.
  • Overview and scheduling of on‑call.
  • Provide coordination of patient care to ensure continuity and quality care.
  • Deliver skilled services efficiently and effectively to meet patient's and family's/caregiver’s needs.
  • Coordinate and review Home Health and Hospice referrals with IDT team.
