Home Care Assessor Willmar, MN

Job Overview

We are a well‑established home‑care agency serving clients in Minnesota since 2007. We are seeking a Registered Nurse (RN) to conduct non‑medical, in‑home assessments for elderly clients in and around the Willmar area (within a 2–3 hour drive).

• Conduct in‑person assessments at the client’s home, discussing non‑medical care needs such as bathing, dressing, grooming, and light housekeeping.
• Complete required health care assessment documents.
• When assigned to assessments for Veterans Affairs (VA) clients, complete the assessment within 3–5 calendar days of receiving notification.

• Active and valid RN license in Minnesota.
• Ability to travel up to 3 hours from Willmar.
• Good communication skills and ability to work independently.

• Assessment duration: 30–60 minutes.
• Compensation: $60 per hour for assessment time plus paid mileage for travel to and from the client’s home.
• Flexible schedule: 8:00 am–8:00 pm, Monday–Sunday; weekend‑only work is acceptable.

Submit a Tuberculosis (TB) skin test if not completed recently. The test can be obtained at a local County Health Department (cost $25) or at a CVS Minute Clinic (cost $75, reimbursement provided).
