Bethesda At Home is seeking a Home Health RN Case Manager in Willmar, MN. The role supports the delivery, coordination, and monitoring of home health services to agency clients, ensuring safe, high-quality care in compliance with regulatory requirements.
Hours are Monday through Friday, 8:00 a.m. to 4:30 p.m. with on-call rotation. Requires RN license in Minnesota and prior home health nursing experience is preferred. Strong communication and organizational skills are essential.
