The MnCHOICES Assessor plays a vital role in supporting individuals and families in Kandiyohi County by conducting comprehensive assessments to determine eligibility and service needs for home and community-based programs. This position is part of an interdisciplinary team that includes professionals with expertise in developmental disabilities, nursing, mental health, aging services, and physical disabilities.
Hiring range: $78,001 - $87,236. Full range: $78,001 - $109,112.
Responsibilities- Conduct MnCHOICES assessments for individuals seeking services through programs including DD, EW, AC, CADI, CAC, BI, CFSS, and Rule 185.
- Complete comprehensive assessments to evaluate functional needs, eligibility, and service requirements for individuals receiving home and community-based services.
- Work collaboratively within an interdisciplinary team of professionals in developmental disabilities, nursing, mental health, aging services, and physical disabilities.
- Maintain accurate, timely, and detailed electronic documentation in compliance with state and county requirements.
- Coordinate with individuals, families, providers, and community partners to gather information and support service planning.
- Ensure assessments and related paperwork are completed within required timelines and standards.
- Utilize electronic assessment tools and related software systems proficiently and efficiently.
- Participate in required trainings, certification processes, and ongoing professional development.
- Maintain confidentiality and professionalism in all interactions and documentation.
- Perform other duties as assigned.
- Bachelor's degree in social work or a closely related field.
- Bachelor’s degree in nursing with a public health certificate or a closely related field.
- Current license as a registered nurse.
- Experience working with individuals with developmental disabilities, mental health needs, aging populations, or physical disabilities (preferred).
- Valid driver’s license and reliable transportation.
- Must successfully pass required background checks and state requirements.
- Strong knowledge of home and community-based services and long‑term care programs.
- Ability to conduct thorough assessments and make informed recommendations regarding service needs.
- Excellent verbal and written communication skills.
- Strong organizational skills with ability to manage a high volume of assessments and documentation.
- Ability to work independently while collaborating effectively within a multidisciplinary team.
- Proficiency in electronic assessment tools, databases, and standard office software.
- Ability to maintain accuracy and attention to detail in a fast‑paced environment.
- Commitment to professionalism, confidentiality, and person‑centered practices.
- Must successfully complete and pass the MnCHOICES Certified Assessor Training (MNCAT) and certification process.
- Ability to sit, stand, and move throughout the workday to perform office and field-based responsibilities.
- Ability to travel within the county for assessments, and potentially outside the county for meetings and/or training.
- Must be able to lift and carry files, laptops, or materials up to 25 pounds occasionally.
- Ability to perform the essential duties of the position with or without reasonable accommodation.
