JOB DESCRIPTION

Job Summary:

This position is responsible for providing clients and family members with support and information with Bethesda Home Health and Bethesda Hospice. This role provides the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising family care givers, providing patient education and counseling, and making referrals for other services. This position oversees and coordinates the admission process.

The social worker serves as the client and family liaison. This position oversees the grievance process and is responsible for thorough follow-up of all concerns. This position assists the director with overseeing the quality assurance program, ensuring that high quality care and services are being provided to the clients of Bethesda Home Health and Bethesda Hospice and clients and their families are satisfied with the care and services provided.

Other duties include managing the organization’s CRM platform.

  • B. Interrelationship with: MSW, Hospice Clinical Nurse Manager, Staffing Coordinator, RNs, LPN, and entire home health and hospice team, Welcome Center, Chaplains, Administration

C. External Contacts:
Visitors, families, other health service providers, potential clients and families, other servicing agencies

Hours of Work:

Salaried/exempt position – typically Mon. – Fri., 8:00 AM to 4:30 PM. Extended/additional hours as needed.

Essential Requirements of Work:

A. Education and Training:
  • 1. A bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, social work, human services, healthcare administration, and psychology.
  • 2. Must be licensed by the state of Minnesota.
  • Experience:
  • Previous social work or gerontology experience is desirable.
  • Personal:
  • Excellent oral and written communication skills.
  • Ability to adapt to differing customer demands.
  • General sales and assessment skills based on consumer needs and satisfaction.
  • An understanding and desire to promote quality principles.
  • Desire to continue to learn.
D. Job Knowledge (Specific)
  • Knowledge of social work practice.
  • Knowledge of the health care delivery system and broad base knowledge of community/state services available for referral.
  • Working knowledge of general medical problems of the geriatric disease process, required for accurate assessment and evaluation of incoming clients.
  • Knowledge of the legal system as it pertains to the protection of vulnerable adults and preservation of rights under law.
  • Knowledge of the assistance system and programs developed to provide financial assistance.
Physical Demands of the Job:
  • Light duty position with prolonged periods of sitting and walking.
  • Prolonged periods of using computer keyboard/laptop
