JOB SUMMARY:

The Program Coordinator provides overall leadership and direction for the Journey to Independence program, offering direct support to Transition Facilitators (TF), the Lead Family Coordinator (LFC), and the Peer Support Specialist. Responsibilities include facilitating referrals, managing program publicity, and coordinating the Advisory Council, in collaboration with the Transition Services Supervisor, to strengthen the System of Care (SOC) and promote successful youth transitions to adult services.

The Program Coordinator serves as the primary contact for SAMHSA and works closely with the PACT Director, Transition Services Supervisor, and other PACT staff to achieve project goals.

SPECIFIC RESPONSIBILITIES:
  • Support transition-aged youth (14-21 years old) in setting personalized goals using the Transition to Independence (TIP®) Model, preparing them for adult roles and increasing positive outcomes during their transition from youth to adult services.
  • Provide guidance to Transition Facilitators, Lead Family Liaison, and Peer Specialist through one-on-one support and team meetings alongside the Transition Services Supervisor.
  • Foster a professional, respectful working environment with positive interactions among participants, staff, community stakeholders, and supervisors, while remaining receptive to feedback.
  • Coordinate program activities with other PACT programming to support the agency’s mission.
  • Work with the Evaluator to collect required data and performance measures, ensuring reports are completed accurately and on time.
  • Serve as the lead staff member to facilitate the goals of the Advisory Council, in collaboration with the Transition Services Supervisor.
  • Receive training in the TIP® Model and apply its practices effectively.
QUALIFICATIONS AND PRIOR EXPERIENCE

REQUIRED:
  • Bachelor’s degree in social work, Human Services, Psychology, or a related field (master’s degree preferred for advanced knowledge in mental health and social services).
  • Relevant life experience (5+ years) may substitute for formal education. Candidates with lived experience working within systems of care, or personal experience navigating and overcoming complex systems, are highly encouraged to apply.
  • Experience Requirement:
    One year of experience working with youth with Serious Emotional Disturbance (SED) or Serious Mental Illness (SMI) is required at the bachelor’s level. No direct experience is required for candidates holding a master’s degree.
  • Ability to pass a background check.
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
  • Familiarity with mental health, social services and county resources within the State of Minnesota and the five counties served by PACT for Families Collaborative.
  • Strong Leadership and team management abilities.
  • Strong communication skills and ability to work independently and as part of a team.
  • Culturally responsive service delivery.
  • Valid driver’s license and car insurance for regional travel.
  • Flexibility for evening and occasional weekend hours.
TRAVEL:

Extensive travel within the five-county service area and occasional statewide/national travel.

Job Type: Full-time

Pay: $54,415.00 - $61,566.00 per year
