Reserve Care provides comprehensive hospice, palliative, and grief support to patients and families.
What You Will Do- Provide personal care and assist with ADLs according to the patient’s plan of care.
- Provide a safe environment for patients.
- Safely ambulate/transfer patients as appropriate.
- Assist with passive and active ROM exercises as directed by the nurse.
- Prepare nutritional snacks as appropriate for the patient.
- Under the supervision of the nurse, assist patient with pre‑poured medications that are ordinarily self‑administered and prescribed by a physician under the supervision of a nurse (Home Care only).
- Under the direction of the nurse, perform simple wound care, skin care, and bowel care protocols according to the plan of care where appropriate.
- Recognize and promptly report any changes in the mental or physical status of the patient to the nurse.
- Record activities provided and observations of patient at the time of visit in clinical records and appropriate documents.
- Complete all charting, timesheets, and mileage logs as defined by agency policy and procedures.
- Attend and actively contribute during patient care conferences.
- Participate in HNA discipline meetings, team meetings, staff meetings, retreats, and in‑services as appropriate.
- Attend in‑services to obtain 3 education hours per quarter.
- Participate in the on‑call rotation as scheduled (Home Care only).
- Assist the Team Leader with other designated activities as necessary (scheduling, supply organization, answering office telephone, etc.).
- Demonstrate positive customer‑service responsiveness according to agency standards.
- Be flexible in meeting patient/family needs that require variable shifts and weekends.
- Participate in chart review quality improvement activities and other meetings as assigned.
- Participate in coordination of and communication with volunteer staff.
- Provide basic caregiving education to caregiver/family members, such as turning and positioning, transfer techniques and changing linens.
- Maintain Transmission‑Based Precautions.
- Complete annual competency requirements.
- Proficiency in reading and writing English.
- State‑tested nursing assistant certification in good standing.
- Equivalent education/experience may substitute for minimum qualifications, except where legal requirements exist (e.g., license).

- Compliance with annual flu shot policy or exemption documentation.
- Active auto insurance policy as a licensed driver in Ohio with required coverage.
- Active driver’s license.

- High level of integrity, ethics, and professionalism.
