Step Down Navigator (Social Work)

Full Time (No weekends)

The Step Down Navigator is responsible for meeting all eligible patients to discuss the benefits of outpatient services as part of your continuum of care treatment. The Step Down Navigator will be responsible for communicating eligibility to key members of senior leadership and associated departments and will be responsible for rountinely collecting and entering data, specific to outpatient eligibility, into Midas for trending report purposes.

The individual will have a thorough understanding of the Step-Down Playbook, a thorough understanding of the inpatient workflow process, as it pertains to case management and discharge planning. This individual can facilitate groups, assist with continued stay reviews and float to the inpatient units as needed.

Master's degree in Social Work, counseling, or a related mental health field from an accredited college or university. Candidate must possess a LSW, LISW, LPC or LPCC issued by the State of Ohio, to qualify for the position.

Two years previous experience in a mental health or substance abuse treatment facility with direct experience with group therapy, crisis intervention, and treatment planning required.

Typical office setting, work environment can be stressful at times, may spend a lot of time on your feet while working with patients, continuous low voices and office machine noises.

This individual must possess excellent verbal and written communication skills. Diplomacy and discretion are essential. This individual must have the ability to prioritize multiple projects. Professional demeanor and appearance required.
