Garden Spring Nursing and Rehabilitation Center is seeking a knowledgeable and detail-oriented RN LNAC (Licensed Nurse Assessment Coordinator) to join our clinical team on a part-time basis
. The RN LNAC plays a key role in coordinating the Resident Assessment Instrument (RAI) process
, ensuring accurate and timely completion of MDS assessments
, and supporting care planning in accordance with federal and state regulations. This position is essential to maintaining regulatory compliance and promoting high-quality resident outcomes within our skilled nursing facility.
- Coordinate and complete MDS assessments in accordance with RAI guidelines and regulatory timelines
- Ensure accuracy and completeness of resident assessments and supporting documentation
- Collaborate with interdisciplinary team members to develop and update resident care plans
- Monitor and track assessment schedules to ensure timely submissions
- Review clinical documentation to ensure accuracy and regulatory compliance
- Assist with Medicare and Medicaid case mix management and reimbursement optimization
- Communicate with nursing leadership regarding assessment findings and clinical changes
- Support survey readiness and compliance with CMS and state regulations
- Utilize electronic medical record systems for documentation and reporting
- Active Registered Nurse (RN) license in Pennsylvania
- LNAC certification or MDS experience preferred
- Previous experience in MDS coordination, case mix, or clinical assessment in a skilled nursing facility preferred
- Strong understanding of RAI guidelines and CMS regulations
- Excellent attention to detail and organizational skills
- Ability to collaborate effectively with interdisciplinary teams
- Proficiency with electronic medical records (EMR) systems
