Garden Spring Nursing and Rehabilitation Center is seeking an experienced and motivated Clinical Liaison to serve as the primary clinical and referral resource for our facility and affiliated centers.

This position will be based at Garden Spring, working Monday through Friday, with occasional weekend availability required to support admissions and census growth efforts. The Clinical Liaison will play a critical role in reviewing referrals, coordinating admissions, and ensuring appropriate clinical placement across multiple centers.

• Review referral portals for multiple centers to evaluate potential admissions

• Conduct detailed clinical reviews to determine appropriateness for skilled nursing placement

• Communicate with hospitals, case managers, and referral sources regarding admission status

• Assess insurance coverage and demonstrate strong insurance awareness (Medicare, Managed Care, Medicaid, and commercial plans)

• Verify authorizations and collaborate with business office and admissions teams

• Maintain timely and accurate documentation within EMR systems

• Utilize and navigate systems such as Wells Sky
  , Repisodic
  , and other referral platforms

• Coordinate with facility leadership to support census development and strategic growth

• Participate in interdisciplinary communication to ensure smooth transitions of care

• Be available for occasional weekend coverage to review urgent referrals

• Clinical background required (RN, LPN, or relevant clinical degree preferred)

• Prior experience in clinical review, admissions, or case management strongly preferred

• Strong understanding of skilled nursing level-of-care criteria

• Knowledge of Medicare, Managed Care, and insurance authorization processes

• Experience with EMR systems required

• Familiarity with Wells Sky and Repisodic preferred

• Excellent communication and relationship-building skills

• Strong organizational skills and ability to manage multiple referral sources

• Monday–Friday schedule with limited weekend requirement
