Garden Spring Center is seeking a detail-oriented and dedicated Licensed Nurse Assessment Coordinator (LNAC) / LPN MDS Coordinator to join our team. This role is responsible for coordinating and completing the Minimum Data Set (MDS) assessments in compliance with federal and state regulations, ensuring accurate reimbursement and high-quality resident care.
Key ResponsibilitiesCoordinate and complete MDS assessments (OBRA, PPS, and managed care) in a timely and accurate manner
Ensure compliance with all CMS, state, and facility guidelines
Collaborate with interdisciplinary team members to gather and verify resident data
Monitor and track assessment schedules to meet required deadlines
Participate in care planning and ensure MDS reflects individualized resident needs
Maintain thorough documentation to support coding and reimbursement
Assist with audits and implement corrective actions as needed
Stay current on regulatory changes and MDS best practices
Active LPN license in good standing
Certification as a Resident Assessment Coordinator (RAC-CT) preferred (or willingness to obtain)
Previous MDS experience in a skilled nursing facility strongly preferred
Strong knowledge of CMS guidelines and reimbursement systems
Excellent organizational, communication, and analytical skills
Ability to work independently and as part of a team
